We’ve covered the most romantic moments on Grey’s Anatomy, the best weddings, and the most satisfying breakups, of course. But today we’re diving deeper into something Shonda Rhimes has always done particularly well: representation.

Grey’s Anatomy has consistently given us some of television’s most memorable queer storylines. The show has explored characters across different sexualities and identities, from coming out journeys to long-term relationships, marriages, and everything messy in between. Some queer couples have given us iconic love stories, while others… well, let’s just say we’re still recovering from Leah.

17. Leah Murphy & Arizona Robbins

Honestly, the final spot on this ranking was an easy choice. Arizona (Jessica Capshaw) and Leah’s (Tessa Ferrer) relationship was messy from the start, plagued by uneven power dynamics and enough drama to leave everyone involved worse off.

It all began on the night of the hospital’s fundraising gala, when Arizona and April Kepner (Sarah Drew) bonded over drinks in a storage closet. Too intoxicated to drive, they called a cab, but the evening didn’t quite end there. Concerned about her friends, Leah later checked in to make sure they got home safely and eventually stopped by Arizona’s apartment. The pair spent the night talking and watching movies, although Arizona remembered very little of it the next day. Worried about what might have happened, she asked Leah if they’d crossed any lines, and Leah assured her they hadn’t… yet.

Things became more complicated when the two later shared a kiss. Leah quickly developed stronger feelings for Arizona, who initially rejected her advances before eventually reaching out herself with that late-night booty call text. The pair began hooking up casually, but the situation came to an abrupt end when Callie asked Arizona to move back in, prompting Arizona to cut things off.

The fallout was significant. Hurt by how everything unfolded, Leah ultimately filed a complaint against Arizona, highlighting the issues created by romantic relationships between attending surgeons and their subordinates. The complaint led to new hospital policies restricting workplace relationships and sparked major tensions among the staff. Yikes.

16. Lauren Boswell & Arizona Robbins

Arizona had a habit of making questionable romantic decisions, and this relationship is one of the biggest examples. Look, we know we should probably be more upset about this particular affair, but it’s Hilarie Burton. We’re only human.

Lauren Boswell arrived at Grey Sloan Memorial as a talented craniofacial surgeon consulting on a complex case. From the moment she and Arizona had an adorable mix-up at the hospital coffee cart, the chemistry was undeniable. Lauren made no secret of her attraction, openly flirting with Arizona even while acknowledging that she was married.

Things escalated throughout the case, with Lauren continuing to pursue Arizona despite her attempts to keep things professional. Eventually, after Arizona showed her to an on-call room during the chaos of a hospital-wide power outage, the attraction became impossible to ignore. The two shared a kiss and ultimately crossed a line that would change everything.

Unfortunately for them, their affair didn’t stay hidden for long. After accidentally swapping scrubs, Lauren unknowingly wore a top with Arizona’s wedding ring pinned to it. When Callie (Sara Ramirez) spotted the ring, she immediately realized what had happened. And just like that, one of Grey’s Anatomy’s most devastating betrayals was exposed.

Lauren departed Grey Sloan after helping the team navigate the aftermath of the massive storm, never to be seen again. But the damage she’d left behind — particularly to Arizona and Callie’s marriage — would have lasting consequences for years to come.

15. Callie Torres & Erica Hahn

After Callie and George (T.R. Knight) divorced, she entered a friends-with-benefits relationship with Mark Sloan (Eric Dane). During this time, Callie also began growing closer to Erica Hahn (Brooke Smith), and Mark was quick to notice the chemistry between them. He started teasing Callie about their connection, joking about them kissing and asking if she had ever thought about grabbing Erica’s hair during surgery and kissing her. Little did he know, he was basically kicking off Callie’s journey toward understanding her sexuality.

Eventually, Callie and Erica shared their first kiss… and immediately panicked. They decided to take things slowly, but after they slept together for the first time, Callie wasn’t quite having the life-changing experience Erica expected. Erica saw it as a major awakening, even comparing it to the feeling of seeing leaves for the first time. Poetic, but definitely not how Callie was feeling!

Unfortunately, Erica’s reaction created one of the biggest issues in their relationship. She struggled to accept Callie’s uncertainty and dismissed her sexuality by insisting she couldn’t “kind of be a lesbian” — a moment many fans identified as clear biphobia and bi erasure. Not impressed, Erica. Shortly after, Erica left Grey Sloan Memorial and never returned, thankfully.

14. Eliza Minnick & Arizona Robbins

When Eliza Minnick (Marika Dominczyk) arrived at Grey Sloan Memorial Hospital as a consultant tasked with overhauling the surgical residency program, she immediately ruffled feathers. Her teaching methods clashed with the attendings, her confidence rubbed people the wrong way, and before long, she had managed to alienate almost the entire hospital.

Well, almost everyone. Arizona was one of the few people willing to give Eliza a chance, and it didn’t take long for sparks to fly between them. While the rest of Grey Sloan was busy plotting Eliza’s downfall, Arizona found herself drawn to the ambitious consultant and her unwavering belief that things could be done differently.

Unfortunately, their relationship never really had the opportunity to fully take off. Between hospital politics, staff rebellions, and the ongoing battle over the residency program, the pair’s romance was constantly interrupted. And just as things seemed to be gaining momentum, Eliza was fired, bringing their relationship to an abrupt end.

13. Callie Torres & Penny Blake

The fandom is pretty hard on this couple — and on Penny (Samantha Sloyan) as a character in general — and we can’t really blame them. Penny was one of the doctors at the hospital who failed to save Derek Shepherd (Patrick Dempsey). To be fair, she did suggest ordering a CT scan, but her superior shot the idea down. Later, she ended up getting a job at Grey Sloan Memorial. Yikes.

When Penny arrived at a dinner party with Callie, Meredith (Ellen Pompeo) immediately recognized her, and the truth about her role in Derek’s death quickly came out. Despite her friendship with Meredith, Callie continued dating Penny and even defended her when tensions arose at work.

Things became even messier when Penny received a prestigious opportunity to work in New York for a year. Callie decided she wanted to go with her, seemingly assuming she could simply take Sofia along. That decision sparked one of Grey’s Anatomy’s ugliest storylines with a devastating custody battle, ultimately resulting in Arizona being awarded full custody.

Callie then ended her relationship with Penny because she couldn’t bear the thought of leaving her daughter behind. However, in a move that still frustrates many fans (aka us), Arizona later adjusted the custody arrangement and allowed Callie to take Sofia to New York so she could be with Penny.

Honestly? It felt unnecessary, especially since Callie likely wouldn’t have extended Arizona the same grace. It also ended up feeling somewhat pointless, considering Callie and Penny broke up not long afterward… yet Callie stayed in New York anyway. A truly baffling sequence of events.

12. Monica Beltran & Amelia Shepherd

This queer Grey’s Anatomy couple is a true case of what could have been. Monica Beltran (Natalie Morales) joined Grey Sloan Memorial as the latest queer attending pediatric surgeon. From the moment she arrived, she caught Amelia Shepherd’s (Caterina Scorsone) attention, but unfortunately, Monica was in the middle of a messy divorce and wasn’t ready to jump into anything.

Once the divorce was finally behind her, Monica found herself in another complicated situation when she hooked up with Winston Ndugu (Anthony Hill), who had also recently finalized his own divorce. Things got awkward when Winston later returned a piece of red lingerie to Monica that wasn’t even hers. Naturally, Monica immediately tried to explain herself to Amelia, who was less suspicious and more entertained by the entire situation.

Over time, Amelia and Monica developed a genuinely sweet connection. They supported each other through difficult cases, bonded over their shared experiences, and spent time together eating Chinese food — basically ticking every box for a potential friends-to-lovers storyline.

Unfortunately, fans never got to see where this relationship could have gone. Just as Amelia and Monica seemed to be heading toward something more, Monica’s story came to a tragic end when she died in an OR explosion. A devastating loss and one of Grey’s Anatomy’s biggest missed opportunities for queer representation. RIP girlie.

11. Levi Schmitt & Nico Kim

Now this is a couple that can truly divide the fandom. Let’s start with the good of Shmico, as they’re affectionately known by fans. This was Grey Sloan Memorial’s first prominent gay male romance, and Levi (Jake Borelli) went through a major coming-out journey while falling for Nico (Alex Landi), the charming but often emotionally guarded orthopedic surgeon.

The two gave us some genuinely memorable moments, including that steamy ambulance scene as well as plenty of sweet and romantic moments throughout their time together. Nico would refer to Levi as “Glasses,” which was too cute.

However, Shmico was also defined by plenty of frustrating on-again, off-again drama. Nico often struggled to show up for Levi emotionally, and there were moments where his treatment of Levi crossed from guarded into genuinely hurtful. While there was clearly chemistry between them, it often felt like Levi was giving much more than he was receiving.

Ultimately, Nico’s career and ambitions would always come first, leaving Levi to question whether he was truly being valued in the relationship. While Shmico had some beautiful moments, the inconsistency is what keeps them from ranking any higher.

10. Teddy Altman & Allison Robin Brown

Okay, okay, this one is a little cheeky since Allison (Sherri Saum) was never technically a present-day character on the show. However, we did get to explore their relationship through a flashback episode during Teddy’s emotional breakdown, and Allison is such a significant part of Teddy’s (Kim Raver) history — especially when it comes to understanding her relationship with Owen (Kevin McKidd) — that we’re making an exception.

Teddy met Allison after Allison nearly ran her over with her bike. The two immediately connected, with Allison buying Teddy a bagel and eventually inviting her to move in with her and her girlfriend, Claire Williams (Rya Kihlstedt). However, things quickly became complicated when Teddy and Allison began an affair, all while Teddy was grieving the loss of her mother. Years later, Teddy would still describe Allison as her “favorite person.”

Tragically, Allison died during 9/11, and her death shaped Teddy’s life forever. In fact, Teddy and Owen named their daughter after Allison, which would be incredibly sweet if Teddy had actually told Owen the full story behind the name.

This relationship was a huge moment in Teddy’s journey, marking her first queer love and a loss she never truly moved past. Of course, it wasn’t without complications: it began as an affair, and we later learned that Allison’s girlfriend Claire knew what was happening the entire time.

9. Mika Yasuda & Taryn Helm

Honestly, it took long enough for Taryn (Jaicy Elliot) to get her own love story. For a long time, she was quietly pining after her very straight mentor, Meredith. Then, in a funny twist of fate, Taryn became the boss to her own crush: Mika (Midori Francis).

The two finally crossed the line at the intern party when Mika invited Taryn upstairs under the very questionable excuse of wanting to see the hole in the room. One thing led to another, and the pair hooked up.

Things got a little complicated when Taryn went to Teddy with concerns that Mika was burning herself out. Mika was frustrated that Taryn had gone behind her back and told her she just needed some space to be mad at her for a while. However, later that night, Mika showed up at the bar with a very important question: Would Taryn be her date to the upcoming wedding?

Taryn pointed out that she was about to become Mika’s boss, but that technically wouldn’t happen until Monday, and the wedding was that weekend. So, she accepted.

One of their most memorable moments came when their very enthusiastic relationship kept Lucas (Niko Terho) and Simone (Alexis Floyd) awake thanks to some extremely loud noises coming from their room. A classic Grey’s Anatomy houseshare drama.

Ultimately, the pair decided to end things when Mika realized neither of them could fully prioritize the relationship while Taryn was her superior. Despite their feelings, they handled the breakup with a surprising amount of maturity and respect.

8. Toni Wright & Amelia Shepherd

Toni Wright (Jen Landon) is a highly skilled plastic surgeon who joined Grey Sloan Memorial in Season 22 and was immediately paired with Amelia for a complex case. At first, Toni pretended she didn’t remember Amelia from medical school, forcing Amelia to confront her about it. However, it was eventually revealed that they were part of the same study group… and Toni had actually avoided Amelia because she had a huge, unrequited crush on her.

The best part? Amelia admitted she had a crush on Toni too, but was too nervous to make a move. Cue the screaming.

It took a little time (and a lot of flirting), but the two finally gave in and hooked up. However, during their night together, Toni started receiving multiple calls from her ex-wife, Celine. She ignored the first few, but eventually decided to answer in case it was about their son, Zachary.

This understandably made Amelia feel insecure, especially after learning Toni was previously married to such a decorated doctor. Toni quickly reassured her in the most Toni way possible — by presenting Amelia with two homemade awards. The first was for the best person, doctor, and lover, and the second was for best ass. Honestly? A well-deserved honor.

Things got more complicated when Celine revealed she wanted to get back together. Toni began pulling away from Amelia while she figured out what she truly wanted. Celine even offered to move back to Seattle, which tempted Toni because it would be the first time Celine had made a major sacrifice for someone else. But ultimately, Toni realized that their son deserved better than watching his parents stay trapped in an unhappy marriage and that she had been unhappy for a long time, too.

Toni finally went to Amelia to confess that she was in love with her and wanted to be together… only to find Cass (Sophia Bush) in Amelia’s bed. Yikes.

We’re not including Cass and Amelia on this list (yet), because at this point, it’s more of a one-time complication than an actual relationship, but it certainly made for a brutal interruption to what could have been a very promising romance. We’ll have to wait for Season 23 of Grey’s Anatomy to find out how that conversation goes.

7. Carina DeLuca & Arizona Robbins

In a storyline that almost feels like a callback to Derek and Meredith’s iconic meet-cute, Arizona and Carina (Stefania Spampinato) first crossed paths at a bar before heading home together for a very memorable one-night stand. Their evening was hilariously interrupted by Carina’s brother, Andrew DeLuca (Giacomo Gianniotti), but that didn’t stop the sparks from flying.

Things got even more interesting when Arizona discovered that Carina was conducting research at Grey Sloan Memorial. The flirtation continued at work, and Arizona later described their chemistry as so good it was practically pain-relieving. Honestly? We get it.

The relationship took a more serious turn after Arizona lost a patient and began pulling away from Carina. A conversation between them inspired Arizona to pursue a new surgical project, and once she’d written up a proposal, she asked Carina to join her on it. Naturally, they sealed the partnership with a kiss.

As their relationship developed, Arizona began to worry about Carina’s free-spirited nature. After Carina casually mentioned that she’d move countries without much hesitation, Arizona wondered whether she was destined to wake up one day and find that Carina has disappeared from her life. Carina reassured her that she had no plans to leave Seattle or end their relationship.

Ironically, it was Arizona who ultimately left. When she decided to move back to New York with Sofia, seemingly to give things another shot with Callie, her relationship with Carina came to an end. While they weren’t together for long, they were undeniably fun to watch, serving up some of Arizona’s best post-Callie chemistry.

6. Cass Beckman & Teddy Altman

Another One Tree Hill girlie making her way onto Grey’s Anatomy! This time, she stuck around for much longer than one chaotic on-call room encounter. Sophia Bush plays Cass Beckman, who isn’t technically a doctor at Grey Sloan Memorial with the rest of the team. Instead, she’s an attending trauma surgeon at Seattle Presbyterian Hospital — although we’re still holding out hope that she’ll eventually make her way over to Grey Sloan.

Cass enters the Grey’s Anatomy universe through her husband, David Beckman, who does work at Grey Sloan. The two have an open marriage, and Cass explores relationships with multiple genders within that arrangement. She immediately has chemistry with Teddy, but once she discovers Teddy and Owen’s marriage isn’t open, she respects their relationship and backs off.

When Teddy and Owen later decide to open their marriage, Cass finally gets the girl, and she and Teddy begin a casual relationship that continues even after Teddy and Owen separate.

However, the Grey’s Anatomy Season 22 finale throws everything into chaos. Teddy and Owen decide to head to Paris together, seemingly reunited once again. Meanwhile, Cass ends up naked on Amelia’s couch… but that’s a whole other story.

Regardless of where things go next, Cass and Teddy were a genuinely fun pairing. Their relationship also gave the show a chance to explore ethical non-monogamy in a way that felt respectful and refreshing.

5. Mika Yasuda & Jules Millin

From besties to something more? Mika and Jules (Adelaide Kan) always had a playful, flirtatious banter as fellow interns, and it was clear there was something simmering beneath the surface. After losing a patient together, the two nearly kissed, and they eventually crossed that line in the locker room, only for Jules to playfully pull away and leave.

The next episode, they picked up right where they left off, sharing a passionate kiss in the elevator. Unfortunately for them, Amelia interrupted the moment. Mika immediately panicked, but honestly… it was Amelia. If anyone was going to see the humor in the situation, it was her.

Things took a more serious turn when Mika’s younger sister was diagnosed with colorectal cancer at just 22 years old. When Jules asked if there was anything she can do, Mika admitted she wasn’t sure she could handle whatever was beginning between them right now, but she could use a friend. Jules promised to be there, and the two settled into a friendship built on genuine care and support.

They remained close until Mika eventually left the show following the heartbreaking death of her sister.

Could this have become something more? Absolutely. The chemistry was there, and fans were ready. But there’s also something really special about the fact that Jules became exactly what Mika needed in that moment: not a romantic complication, but a true friend.

4. Chaplain James Morgan & Levi Schmitt

Finally, Levi got the happy ending he deserved, and it came with the hospital chaplain. Iconic. After everything he went through with Nico, seeing Levi find a relationship built on kindness, honesty, and genuine support was exactly what fans wanted for him.

The pair hit an early bump when Levi discovers James’ wedding album. James (Michael Thomas Grant) explained that his husband, Ryan, had died and that he hadn’t known how to bring it up. Not cheating, just widowed. The two were able to move past the misunderstanding, opening up about their feelings and having a much-needed conversation about their pasts.

When Levi was offered a pediatric position in Texas, he faced a difficult decision. Despite also being offered a general surgery role at Grey Sloan, Levi chose to follow his true passion. After discussing the possibility of long distance, Levi asked James to move to Texas with him and admitted that he was falling in love with him.

After taking like one day to think it through, James agreed to start this next chapter together, immediately quitting his job to make the move happen. We hope they’re still very happy in Texas!

3. Maya Bishop & Carina DeLuca

Does this count as a queer Grey’s Anatomy couple? We love them, so we’re saying yes.

Carina first appeared on Grey’s Anatomy as Andrew DeLuca’s older sister and an OB/GYN at Grey Sloan Memorial, famously introduced while studying the science of orgasms. However, her biggest storyline — and her well-deserved happy ending — came through the spinoff series Station 19.

Carina met Maya Bishop, a fiercely ambitious fire lieutenant, and the two quickly developed one of the franchise’s strongest queer romances. Their relationship grew into a passionate, supportive, and enduring marriage built on genuine love and chemistry.

What makes Carina and Maya stand out is how refreshingly healthy their relationship is. In a universe that has often put queer couples through heartbreak, betrayal, and complicated drama, they represent something different: a sapphic love story filled with joy, commitment, and the kind of everyday partnership fans love to see.

2. Amelia Shepherd & Kai Bartley

Who can resist a later-in-life discovery of sexuality? We certainly can’t! Kai (E.R. Fightmaster) was Amelia’s first queer romance, and honestly, it was a storyline we were so excited to see.

Kai was a brilliant nonbinary researcher who met Amelia while they were working together on a patient case. This was Amelia’s first relationship after Link (Chris Carmack), and while the two share a child in Scout, we were more than ready to see Amelia move on romantically. Link and Amelia may have created a beautiful family, but their relationship had run its course.

Kai and Amelia were a breath of fresh air. Their romance was a slow burn built on mutual respect, intellectual connection, and sizzling chemistry. We also loved seeing a nonbinary character at the center of a romantic storyline and getting the love story they deserved.

Their biggest challenge came when Kai met Scout and admitted they didn’t want children. However, Kai’s perspective began to shift when they realized this wasn’t about becoming a parent in the traditional sense, this was about loving and supporting the child of someone they loved.

Sadly, their romance was cut short when Kai received a once-in-a-lifetime career opportunity… in London. This is why we can’t have nice things!

1. Callie Torres & Arizona Robbins

Love them or hate them, you can’t deny the impact of Calzona. They were one of Grey’s Anatomy’s longest-running LGBTQ+ couples and completely groundbreaking when it came to queer representation on the show. We got to watch two women fall in love, get married, have a child, navigate heartbreak, survive an amputation, and face some of the biggest challenges the series threw at them.

Were they perfect? Absolutely not. Their relationship had some incredibly toxic moments, including infidelity and painful betrayals. But that’s also what made them feel so significant — Callie and Arizona were allowed to be messy, complicated, and fully developed characters beyond just being a queer couple.

The pair’s story began with a kiss in the bar bathroom, but things quickly got complicated when Arizona discovered Erica had been Callie’s first girlfriend. Arizona explained that she spent her work life caring for newborns and didn’t want to deal with them in her personal life — a comment that definitely didn’t go down well. Things got even more awkward when Callie realized Arizona had actually been at the bar on a date with another woman. Oops.

Eventually, Arizona changed her mind, and the two officially began dating. They briefly broke up when Arizona accepted the Carter Madison Grant and moved to Malawi to help children in need, but she returned after realizing how much she missed Callie.

Unfortunately, by the time she came back, Callie had moved on in her own complicated way — sleeping with her best friend Mark Sloan (RIP) and discovering she was pregnant. After some major adjustments, the three built a family together, raising their daughter Sofia, before Callie and Arizona eventually got married.

Of course, their story wasn’t without heartbreak. The two eventually separated and went through that messy custody battle. But when we last saw our girls, Arizona was heading to New York to reunite with Callie after her breakup with Penny, leaving fans with just enough hope that maybe, someday, Calzona could find their way back to each other.
